29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On or about June 6, 2016 employees using powered tools such as pneumatic nail guns were not protected against eye injuries while performing roofing operations at a residential property.       Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.     On or about June 6, 2016 employees were not protected from a fall while performing roofing operations at a residential property with unguarded skylights, exposing employees to a falls between 17.5 and 19 feet.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    On or about June 6, 2016 an employee performing roofing operations at a residential property was exposed a fall due to improperly tying off to the harness.  The employee had the lanyard attached to the straps between his inner thighs as the attachment point for the body harness.  Employees were exposed to falls between 17.5 and 19 feet.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
