29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment. Warehouse Area - On or about May 10 through June 6, 2016, order picker operators wore shock-absorbing lanyards 6' in length that were too long to prevent them from hitting the warehouse floor if they were to fall from the order picker platform when picking product at the lower rack height of approx. 5' 9" above ground level. Employees were exposed to fractures and contusions. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.132(f)(1)(iii): Employee(s) required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: Warehouse Area - On or about May 10, 2016 through June 6, 2016, order picker operators wore pack-type shock-absorbing lanyards. The employer's training did not include instructions to attach the shock absorber pack snap hook to the employees' body harnesses and not to the anchorage points on the order picker vehicles, exposing employees to potential injuries in the event of a fall and improper deployment of the shock absorber. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
