29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level: Jobsite: The Werner D1224-2 24-foot Type II Aluminum D-Rung portable extension ladder used by an employee to gain access to trim window was placed on unstable and uneven surface. On or about May 17, 2016, the above ladder was displaced while the employee was descending the ladder, and the employee sustained possible spinal injuries from the incident.
