29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     (a)  At least one (1) employee working from the ground tossed a nail gun to an employee who was working on the second level, and was exposed to head injuries where overhead hazard exists.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)   Employees were performing roofing work were exposed to fall hazard and were not protected by the use of a personal fall arrest system or safety net system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder were not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, were not provided to assist employees in mounting and dismounting the ladder. In no case did the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:    (a)   Employees were using a 16-foot extension ladder that did not extend 3-feet above the landing of the roof.
