29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Warehouse - On or about June 7, 2016, employees retrieving product are disconnecting their fall arrest lanyards while transitioning, at approximately 15 feet above grade, from powered industrial trucks from the upper levels of a metal shelving system. Abatement note: Among other feasible means of abatement, the employer may utilize alternative, engineered attachment devices to include, but not limited to, retractable life lines as part of an improved powered industrial truck fall protection system.
