29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: a) On or about 6/7/2016, on the northwest side of the plant, the employer allowed employees to access a ladderway which lacked a swinging gate for the passage through the railing in order to conduct maintenance. b) On or about 6/7/2016, on the top of the fly ash silo, the employer allowed employees to access a ladderway which lacked a swinging gate for the passage through the railing in order to conduct maintenance. c) On or about 6/7/2016, on the top of the cement silo, the employer allowed employees to access a ladderway which lacked a swinging gate for the passage through the railing in order to conduct maintenance. d) On or about 6/7/2016, on the north side hopper platform, the employer allowed employees to access a ladderway which lacked a swinging gate for the passage through the railing in order to conduct maintenance. e) On or about 6/7/2016, on the south side hopper platform, the employer allowed employees to access a ladderway which lacked a swinging gate for the passage through the railing in order to conduct maintenance.
