29 CFR 1910.242(b): Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment. On May 29, 2016, in the Wrapper Room, a maintenance employee was using a compressed air wand to clean the crumbs out of a conveyor opening on UBE #1 Wrapper and the six inch nozzle used was not reduced to 30 p.s.i. when dead-ended.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Maintenance employees performing a servicing activity on #1 UBE Wrapper Machine was not required to: (1) turn off and shut down the machine. (2) physically locate and operate energy isolating device, 480 Volt AC, in such a manner as to isolate the machine from the energy source (3) affix lockout devices to each energy isolating device, electrical disconnect switchbox (4) prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine have been accomplished. a. On May 29, 2016, in the Wrapper Room, a maintenance employee was exposed to an amputation hazard from the in-running nip points of the conveyor sprocket while cleaning the inside the conveyor cavity of the #1 UBE Wrapper Machine to clean out the clumps inside the cavity. b. On July 7, 2016, in the Wrapper Room, a maintenance employee was exposed to an amputation hazard from the in-running nip points of the conveyor sprocket while cleaning the inside the conveyor cavity of the #1 UBE Wrapper Machine to clean out the clumps inside the cavity.
