29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder.  (a) Roof - The extension ladder used to access the roof did not extend three feet above the roof line.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) Jobsite - Employees were exposed to fall hazards from the roof to the ground below where a competent person had not adequately inspected the jobsite to ensure it was free from hazards.  Steve Marchand, DBA IRoof, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1926.501(b)(13), which was contained in OSHA inspection number 1055551, citation number 01, item number 001, and was affirmed as a final order on 09/15/15, with respect to a workplace located at 172 Pine Street, Holbrook, Ma. 02343.
