29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 242 Broome Street., NY, NY: On or about 6/6/2016, an employee using a Makita hammer drill / concrete chipper was exposed to potential shock hazards due to the plug end of an electrical extension cord not being provided with strain relief thus exposing the energized conductors.
