29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:       a)  Delancey & Essex Parking Garage, 107 Essex Street, NY, NY: On or about 6/6/2016, employees using a HTC 800 RX Three Phase Remote Controlled Floor Grinder were exposed to potential shock hazards due to the power cord not being provided with strain relief at the point of a repair, thus exposing the energized conductors.            NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
