(a) On or about June, 8, 2016 at worksite 4404 S, Lees Summit, Missouri--employees were exposed to serious eye injury while cutting roofing tile with a TS 420 Stihl Cutquik saw without eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about June 8, 2016, at worksite 4404 Shadow Valley Cir Lees Summit, Missouri.-,an employee was performing roofing operations at an elevated height approximately 25-28 feet above the surface below, without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems. Abatement certification for this violation is not required.
