29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 1 Howard St., NY, NY- 1st floor dining area: On or about 6/7/2016, employees were exposed to potential shock hazards due to energized conductors entering the plug end of an electrical extension cord not being provided with strain relief.
