29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    WORKSITE: The employer did not initiate and/or maintain safety programs which involve performing frequent and regular inspections and assessments of the worksite, materials, and equipment where employees were required to demo and remove old fire sprinkler piping that was not adequately supported and/or secured in place.       On or about 6/2/2016 an employee was attempting to remove an old fire sprinkler pipe from the ceiling when one end of the pipe became detached from its hanger. The pipe struck the stepladder and employee, causing the employee to fall from the stepladder to the floor below. The employee sustained serious injuries from the incident.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  WORKSITE: The employer did not provided specific training and instruction to the employee in the recognition of hazards or the procedures necessary to eliminate or minimize hazards associated with the demolition and removal of old fire sprinkler pipes.    On or about 6/2/2016, the employee was attempting to remove an old fire sprinkler pipe from the ceiling when one end of the pipe became detached from its hanger. The pipe struck the stepladder and employee, causing him to fall from the stepladder to the floor below. The employee sustained serious injuries from the incident.
