29 CFR 1910.23(a)(9): Each floor hole, into which persons could accidentally walk, was not protected by a cover that leaves no openings more than 1 inch wide: a) On or about June 7, 2016 gaps between the loading dock and the trailer were greater than 1 inch where employees entered and exited the trailer for loading purposes and was not covered.

29 CFR 1910.29(a)(4)(ii): Ladder stands did not have at least two (2) of the four (4) casters provided with a positive wheel and/or swivel lock to prevent movement and/or were not of the swivel type: a) On or about June 7, 2016 two mobile ladder stands were observed with the manual brake disabled by the handle taped to the side rail of each stand, exposing employees to fall and/or slip hazards when the mobile ladder stands were in use.
