29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about June 8, 2016, employees were not protected from electrical shock/ electrocution while working within 2.3 feet of an energized service drops operating at 480 volts, the employer did not ensure the lines were de-energized, effectively guarded or the minimum approach distance was maintained. No abatement certification is required of this item.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about June 8, 2016, employees were not protected from a fall while performing commercial roofing operations, exposing employees to a fall of 19.3 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
