29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system :  a- Roof Level: Employees were not wearing personal fall arrest harnesses and exposed to approximately 18ft falls from a 3 on 12 pitched roof. Condition noted on or about 06/03/2016 b- Roof Level: Employee was wearing a personal fall arrest harness but was not secured via an anchor point employee was exposed to falls of approximately 18ft to the concrete below. Condition noted on or about 06/04/2016.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(  a- Roof level: Employees using an extension ladder to access a roof were exposed to fall hazards of approximately 18ft to the concrete below as the ladder side rails did not extend 3ft above the upper landing surface. Condition noted on or about 06/03/2016.
