A) Resnick Distributors, New Brunswick, NJ/ Conveyor Belt System - The equipment was not used in accordance with of the manufacturers UL labeling or listing. The employer exposed the employees to electrical hazards, by connecting relocatable power tap to a Husky power tool extension cord used to power a power tool. Observed on 06/08/2016
