Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure the permit required confined space program was implemented prior to employees entering the 12 Yard Plant Cement Mixer. Employees are required to enter the plant mixer to chip out and replace the polyurethane liners and did not perform air monitoring or lock out the mixer prior to entry.

Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure each employee required to enter a permit required confined space was trained with the knowledge and skills necessary to safely perform entry of a permit required confined space. Employees were not trained in required permit documentation measures, that atmospheric testing was needed when entering and while in the cement mixer, and the need to have an attendant outside the pace to help the entrant in an emergency situation. Employees are exposed to hazards from the rotating mixer and hazards associated with welding fumes.
