29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): The point of operation of the following equipment was not guarded in such a way as to prevent the operator from having any part of their body in the danger zone during the operating cycle: A. The Cincinnati 90-350 ProForm press brake, serial number 55842, located in the press break area was not guarded. Press brake had no guard to prevent access to side and rear of machine and the light curtain system was disabled and had the control keys left in it. B. The Cincinnati 90-350 MaxForm press brake, serial number 54816, located in the press break area was not guarded. Press brake had a side interlocked guard that was in the open position and had the interlock device defeated. The light curtain system was also disabled on this press brake. C. The Accupress 710012 press brake, serial number 3437, located in the press break area was not guarded. Press brake had no guard to prevent access to rear of machine.
