29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) On or about March 25, 2016, Phaze Concrete, Inc. did not ensure that each employee in an excavation was protected from cave-in hazards. The employer did not use a protective system in accordance with OSHA 1926.652(c) standards. This condition exposed employees to a crushing hazard by trench wall cave-in.
