29 CFR 1910.23(a)(4): Skylight floor openings and holes were not guarded by a standard skylight screen or a fixed standard railing on all exposed sides: On or about May 12, 2016 employees working on the roof cleaning the carbon dioxide exhaust stack were exposed to fall hazards because the skylight adjacent to the work area was not protected to prevent employees from falling through it.
