29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: a) At the facility, on or about June 8, 2016 - An employee designated as a machine operator was not refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation. An employee designated as a machine operator had an exposure to continuous noise at a dose of 72.4%. The exposure is approximately 1.44 times the OSHA action level of 85 dBA. This is equivalent sound level of 87.6 dBA.
