29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: a) The Bizerba Slicer s/n 30010905 was not free from recognized hazards in that the slicer had an opening exposing employees to live voltage of 113v. b) The Bizerba Slicer s/n 30010905 was not removed from service when defects were found in that the employees had to use a pen to actuate/to turn it on, exposing employees to live voltage. c) The power cord for the Shrink Wrap machine was plugged into an outlet that was not free from recognized hazards in that the outlet had burn marks and charring.
