29 CFR 1910.1200(e)(1): Employer had not developed, implemented or maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met:      (a) Denver North Companies (Lefty's Front Range Grille) located at 8400 Pena Blvd, Denver, CO: On and preceding 6/9/16: The employer had not developed, implemented or maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of 29 CFR 1910.1200 for labels and other forms of warning, safety data sheets, and employee information and training will be met. Employees used chemicals to wash dishes in the kitchen and bar area. This condition exposed employees to chemical hazards associated with the use of chemicals such as, but not limited to:    1. Oasis 146 Multi-Quat Liquid Sanitizer;  2. Apex Pot and Pan Soak;  3. Jet-Dry;  4. Eco-San;  5. Super Trump.

29 CFR 1910.1200(h)(1): The employer did not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    (a) Delaware North Companies, at 8400 Pena Blvd, Denver, CO: On and preceding 6/9/16 the employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment. Employees used chemicals to wash dishes in the bar and kitchen. This condition exposed employees to chemical hazards associated with use of chemicals such as but not limited to:    1. Oasis 146 Multi-Quat Liquid Sanitizer;  2. Apex Pot and Pan Soak;  3. Jet-Dry;  4. Eco-San;  5. Super Trump.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d):    (a) CA Concessions of Colorado, Inc. dba Lefty's Front Range Grill located at 8400 Pena Blvd, Denver, CO: On and preceding 6/9/2016, the employer did not have verification that the required workplace hazard assessment had been performed as per the requirements outlined in 29 CFR 1910.132(d).     Employees performed operations where they were exposed to chemical hazards, including but not limited to the following:    1.  Exposure to chemical hazards when dishwashing.
