29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On June 9, 2016, on the ground, employees working below the employees on the roof were not wearing hard hats, exposing themselves to an overhead hazard.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:  a. On June 9, 2016, the ladder jack scaffold did not have a guardrail and the employee on the plank board was not attached to a personal fall protection system, exposing himself to a 20 feet fall hazard.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On June 9, 2016, on the 4:12 pitch roof, two employees installing metal sheathing were not protected from a 22 feet fall hazard by the use of a fall protection system.
