29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury: On or about June 6, 2016 at the above addressed jobsite, employees were using a pneumatic nail guns and were not wearing safety glasses, thereby exposing the employees to the hazards associated with eye injuries. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about June 9, 2016 at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three feet above the upper landing surface, thereby exposing employees to the hazards associated with falls. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 9, 2016 at the above address employees were performing roofing work on a residential home. Employees were not utilizing fall protection and were exposed to a fall hazard of approximately 19.6 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Awans Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 995824, citation number 1, item number 1 and was affirmed as a final order on February 4, 2015 with respect to a workplace located at 843 S. Ashland Ave, Lagrange, IL.
