29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 9, 2016 and in times prior thereto the ladder accessing the inside of the trench did not extend 3 feet above the landing, exposing workers to a fall from elevation hazard of approximately 30 feet: a. North trench: The ladder did not reach the top of the trench box. b. South trench: The ladder extended 31.5 inches above the trench box.
