29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   a. In regards to citation 1, item 1, issued on July 22, 2016, the employer has failed to submit abatement certification and documentation within ten days of the abatement date, August 24, 2016.  The violation read as follows:       29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a. The employer did not develop an energy control program to include energy control procedures, employee training and periodic inspections when employees were expected to perform service and maintenance activities, such as but not limited to cleaning or clearing a jam from the Whitaker grinder.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a. The employer did not develop an energy control program to include energy control procedures, employee training and periodic inspections when employees were expected to perform service and maintenance activities such as but not limited to, cleaning or clear a jam from the Whitaker grinder.

29 CFR 1904.7(b)(5): How do I record an injury or illness that involves medical treatment beyond first aid? If a work-related injury or illness results in medical treatment beyond first aid, you must record it on the OSHA 300 Log. If the injury or illness did not involve death, one or more days away from work, one or more days of restricted work, or one or more days of job transfer, you enter a check mark in the box for cases where the employee received medical treatment but remained at work and was not transferred or restricted.   Located at Advanced Technology Products in Milford Center, OH:  a.  On the 2015 OSHA 300 log the employer did not record injuries that involved medical treatment beyond first aid.
