29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place:  In the Storage shed area, an employee operating a Clark forklift (Model C5001Y55) was exposed to struck-by hazards. The employee was exposed in that the control lever(s) were missing markings, to avoid moving the levers in the opposite direction and striking someone or something.
