29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards: At the job site employee(s) and contractor employee(s) were exposed to fall hazards due to the company's safety and health work site rules not being implemented for the following guidelines: (a) Frame sections were not pinned together to prevent them from separating or secured together in some other manner approved by the manufacturer. (b) Scaffold cross bracing were serving as guardrails. (c) A fixed ladder was attached to a scaffold system greater than three tiers high (15 to 18 feet). (d) Internal scaffold stairway systems are mandatory on any scaffold higher than three tiers (15 to 18 feet). (e) The employer did not complete the scaffold inspection checklist prior to tagging the accident scaffold with a green tag. The green tag indicates the scaffold is safe to use by all employees trained in proper scaffold use.
