29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: (a) On or about June 11, 2016, Scheffe Roofing, Inc. did not ensure that eye and face protection were used by employees when machines or operations presented potential eye or face injury. An employee of Scheffe Roofing, Inc. was observed nailing shingles on a residential roof, using a pneumatic gun, without any eye or face protection. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). Scheffe Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 954588, citation number 1, item number 1 and was affirmed as a final order on April 28, 2014, with respect to a workplace located at 789 Black Arrow Dr. Colorado Springs, CO 80921

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or about June 11, 2016, Scheffe Roofing, Inc. did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of Scheffe Roofing, Inc. were exposed to fall hazards in excess of 10 feet while engaged in roofing activities on the roof of a residential building with no fall protection in place. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
