29 CFR 1926.1417(w): The employer did not use a tag or restraint line when necessary to prevent rotation of the load that would be hazardous: On or about June 07, 2016, at the above addressed jobsite, four employees were using a crane to lift a large concrete shoring form, for the purpose of disassembling the form, during which the employees were using their hands, in lieu of a tag/restraint line to guide/control the load, wherein the employees were unable to maintain effective rotational and/or pendular control of the load, and also resulted in exposing multiple employees to struck-by and/or amputation hazards. Commercial Forming Corp South was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.1417(w) and 29 CFR 1926.1428(c)(3), or its equivalent standard, which was contained in OSHA inspection number 316327733, citation number 01, Item number 001b, and was affirmed as a final order on 10/11/2012, with respect to a workplace located at 4100 Williams Island Boulevard Aventura Florida 33160, and contained in OSHA inspection number 315359877, citation number 01, item number 002, and was affirmed as a final order on 10/11/2012, with respect to a workplace located at 30 Andalusia Avenue Coral Gables Florida 33134.

29 CFR 1926.1428(c)(3): The crane signal person did not have a basic understanding of equipment operation and limitations, including the crane dynamics involved in swinging and stopping loads and boom deflection from hoisting loads: On or about June 07, 2016, at the above addressed jobsite, an employee was giving radio signals to the operator of a crane, for which the employer did not ensure that the signal person had a basic understanding of the crane's and a tag line's operational limitations, which resulted in the loss of control of the load (concrete shoring form), and also resulted in exposing multiple employees to struck-by and/or amputation hazards.
