29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii);  (a) Construction Site - On or about June 8, 2016 three employees were working from scaffold platforms that were not fully planked or decked from front to back.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by a competent person:  (a) Construction Site - On or about June 8, 2016, employees had just moved the scaffold from the front of the building to the side without the supervision and direction of a qualified competent person. Employees performing the erection of the scaffold had not been trained.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:   (a) Construction Site - On or about June 8, 2016 employees were working from scaffold platforms that were more that 10 feet above a lower surface. Sections of the scaffold platforms that were being used was missing some of the guardrails. Some horizontal rails were not secured on the ends to the frame.  Employees were not protected from falling by a guardrail system or personal fall arrest system.
