29 CFR 1910.244(a)(2)(i): In the absence of a firm foundation, the base of the jack shall be blocked. If there is a possibility of slippage of the cap, a block shall be placed in between the cap and the load: a. On or about June 9, 2016, on the grounds of the staging yard consisting of gravel, dirt, and vegetation, the employer did not ensure that the base of the 12 ton CARQUEST bottle jack and 3 ton Torin Pro Series jack stand (S/N: 1210003106) were blocked while an employee was changing the rear brakes on a Chevrolet (BCI 14; model: K31043; plate no.: PHQ 5572) utility truck. On or about June 9, 2016, the Chevrolet utility truck (BCI 14) fell on the employee resulting in fatal injuries.
