29 CFR 1917.42(g)(2)(vi): Synthetic web slings were not removed from service when warning threads designed to indicate excessive wear or damage became visible. a) Kinder Morgan Bulk Terminal: On or about 06/13/2016, Subcontractor employees were provided with yellow synthetic web slings with exposed warning thread to offload bulk cargo as a result employees were potentially exposed to possible injuries. No abatement certification or documentation required.

29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees. a) Kinder Morgan Bulk Terminal: Vessel Voulas Seas: On or about 06/13/2016, Subcontractor employees were required to stand on sacks of dry chemical and were not protected from a 7 feet fall to the bottom of the ladder well. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
