29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about June 6, 2016, the employer did not ensure maintenance employees replacing the belts of the Gel Coat Source ventilation fan and electric motor, turned off and shut down the equipment using the established procedures, operated all of the energy isolating devices, and affixed lockout devices to the energy isolating devices. Energy sources include, but not limited to; electric and kinetic power.
