29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    (a) Location - 29 Washington Square North, Salem, MA:  On or about 6/14/16, the employer failed to inspect scaffold brackets that were used on scaffold.

(a) Location - 29 Washington Square North, Salem, MA:  On or about 6/14/16, an employee was exposed to falls when the fall protection anchor was not secured properly to the peak of the roof.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     (a) Location - 29 Washington Square North, Salem, MA:  On or about 6/14/16, an employee was exposed to falls when using a ladder to access the roof and it did not extend three feet above landing.
