A. Stillwater Contracting, LLC worksite located at 740 Williamstown Rd., Ottawa, Ohio: On or about June 10, 2016, the employer did not ensure an employee did not stand on the guardrails of a JLG Aerial Lift, model 460S. The employee was exposed to a potential fall due to the weak working position.

Stillwater Contracting, LLC worksite located at 740 Williamstown Rd., Ottawa, Ohio: On or about June 10, 2016, the employer did not ensure an employees personal fall arrest system was not secured (clipped-off) to the Genie 400S Aerial Lifts. Two employees were performing steel erection activities from the aforementioned aerial lifts.
