29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:   a) Job site, 140 Commerce Drive, Freedom, PA: On or about June 1, 2016, the employer did not ensure that an annual energy control inspection was conducted at the worksite. Deficiencies noted during the inspection included, but were not limited to, not providing all necessary energy control hardware/devices such as air valve covers to perform lockout during preventative maintenance activities on the make on/pull on machine and not ensuring that written machine specific energy control procedures existed for equipment such as the make on/pull on machine.

29 CFR 1910.147(d)(3):  All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):      Instances including, but not limited to, the following:    a) Job site, 140 Commerce Drive, Freedom, PA: On or about June 14, 2016, the employer did not ensure that all energy isolating devices were located and physically operated in such a manner so as to disconnect power to the machine in that during die changing activities on the Landis pipe thread machine, company identification number 003509, the electrical power was left connected, with power on to the machine during the work.
