The employer failed to ensure written lockout/tagout (LOTO) procedures located on machinery were utilized by employees performing mold prep on the MOH 1203 Tilt Mold Table.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about June 8, 2016, the employer failed to ensure the written lockout/tagout procedures on MOH 1203 Tilt Mold Table and MOH 289 Hydraulics Motors identified all sources of energy. Employees were exposed to gravity converted to kinetic energy when a safety bar was not utilized to stop the mold plates from closing during Mold Prep. Park Ohio Products Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii)(B), which was contained in OSHA inspection number 457493, citation number 01, item number 001 and was affirmed as a final order on August 21, 2012, with respect to a workplace located at 7000 Denison Avenue, Cleveland, Ohio 44102.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about June 8, 2016, employees were exposed to machine hazards associated with pressurized hydraulic, pneumatic, electrical, and gravity as sources of energy. The employer failed to implement energy control application steps as the MOH 1203 Tilt Mold Table was not shut down or turned off to perform the servicing work. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)] and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. General Aluminum Mfg Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)9i), which was contained in OSHA inspection number 892433, citation number 01, item number 001B and was affirmed as a final order on August 16, 2013, with respect to a workplace located at 5159 S. Prospect Street, Ravenna, Ohio, 44266. General Aluminum Mfg Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(3), which was contained in OSHA inspection number 943017 citation number 02 item number 001A and was affirmed as a final order on May 17, 2015, with respect to a workplace located at 1370 Chamberlin Blvd, Conneaut, Ohio, 44030.
