29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:        a) On or about 06/14/16, in the Administrative Area 2 Corridor of the Baker Victory Services Residential Treatment Facility under Construction at 150 Martin Rd., Lackawanna, NY:  Two employees were exposed to a fall of 10 ft. 6 in. to the floor while walking on 2x6 horizontal members of trusses above the ceiling in the process of installing electrical conduits.  The truss members were spaced 2 ft. apart. No fall protection system was utilized.         NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) On or about 06/14/16, in the Administrative Area 2 Corridor of the Baker Victory Services Residential Treatment Facility under Construction at 150 Martin Rd., Lackawanna, NY:  An employee used a 10 foot  stepladder to climb onto 2x6 horizontal members of trusses above the ceiling.The employee was exposed to a fall of 10 ft. 6 in. to the floor because the side rails of the stepladder were approx. 6 inches below the 2x6 horizontal members of the trusses.        NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    a) On or about 06/14/16, in the Administrative Area 2 Corridor of the Baker Victory Services Residential Treatment Facility under Construction at 150 Martin Rd., Lackawanna, NY:  An employee was on the top step (ie., the first step below the top) and also stood on the top of a 10 foot stepladder in order to access 2x6 horizontal members of trusses above the ceiling.      NO ABATEMENT CERTIFICATION REQUIRED
