29 CFR 1926.416(a)(1): An employee was permitted to work in proximity to electric power circuits and was not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) On or about June 14, 2016 in the Ter-Jan Carpet Cleaning building attic crawl space, an employee installing air conditioning equipment and working in close proximity to an energized metal conduit was exposed to an electrical hazard.

29 CFR 1926.1203(a): Before it began work at a worksite, the employer did not ensure that a competent person identified all confined spaces in which one or more of the employees it directed may have worked, and did not identify each space that was a permit space, through consideration and evaluation of the elements of that space, including testing as necessary: (a) On or about June 14, 2016 at the Ter-Jan Carpet Cleaning building where the employer did not evaluate the attic crawl space, a confined space, for electrical hazards that could trigger permit required confined space requirements.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about June 14, 2016 at the Ter-Jan Carpet Cleaning building where an employee who was installing air conditioning equipment was exposed to falling through ceiling joist gaps approximately 9 feet above the lower level.
