29 CFR 1926.403(b)(2):  Listed, labeled, or certified equipment were not installed and used in accordance with instructions included in the listing, labeling, or certification:  a)  Basement - On or about 05/20/2016, carpenters were exposed to a 120 volt electrical contact hazard from a outlet box which was left on the floor and not installed in a wall as its listing and labeling intended.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Second Floor - On or about 05/20/2016,  4 foot by 10 foot floor hole which served as an entrance to a set of stairs was not provided with a cover, standard guard rails, nets, or similar fall protection.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1052(b)(3): Treads for temporary service shall be made of wood or other solid material, but were not installed the full width and depth of the stair:   a)  First Floor - On or about 05/20/2016, workers were exposed to a 10 foot fall from a set of temporary wooden stairs having wooden stair treads which only covered 7 inches of the 9 inch stair treads.  b)  Second Floor - On or about 05/20/2016, workers were exposed to a 10 foot fall from a set of temporary wooden stairs having wooden stair treads which only covered 7 inches of the 9 inch stair treads.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
