29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidentally walk were not guarded by a floor hole cover of standard strength and construction:    Location: 37 Christopher Road, Holliston, MA    On or about 6/15/16, the employer did not ensure that walking/working surfaces in the attic were free of unprotected floor openings between the joists, which resulted in an employee fall of greater than 7-feet, 6-inches to the wood floor below.
