Reefer electrical power station; on or about 13 June 2016 and times prior thereto; strain relief was not provided for an energized 240 volt electrical 50 watertight female cord connector routed from an ESL M4-R50-240-50-65SC Power system. This condition exposed employees to an electrical shock hazard.

AML Landing, on or about 13 June 2016; Forklift extensions measuring 180 inches in length by 16 inches in width, were not approved for use by an experienced professional engineer with knowledge of the operating characteristics of the equipment being used; to determine the stability and center of gravity changes when operating with the extensions installed. This condition exposed employees to a struck by and crushing hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  AML Landing, Checker shack, outside wall adjacent to stairs; on or about 13 June 2016 and times prior thereto; an energized 115 volt electrical panel board had open blanks in slots 22 and 24. Open blanks measured 1 1/4 inches by 2 inches. The panel board is used for operating yard lights. This condition exposed employees to electrical shock and electrical burn hazards.    The Arrowhead Transfer Incorporated was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 1020577, citation number 1, item number 2a and was affirmed as a final order on 03 March 2015, with respect to a workplace located at 520 Front Street in Wrangell, Alaska.
