OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to falls from height while using incorrect ladders for tasks: (a) On or about June 8, 2016 - Pilot Line Filler Room, an employee was injured when he was working from the 3rd or 4th rung of an 8-foot A-Frame Portable Reinforced Plastic (PRP) ladder to collect a quality control sample from a vertical pipe by extending his arms overhead and off to the right to reach the vertical pipe cap.
