29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): On or about 6/16/16 at the Wrap Area platform; the employer allowed employees to work from the platform 10.5' high that had to access the main disconnect box for the production department; the platform had no guardrails on the left side and no mid rail on the front side, with a 25" open space between the end of the platform and the wall; thus exposing employees to fall hazards.
