29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. 39 Ludlow St., Newark NJ Multiple employees used vent pipes as an anchorage point for their personal fall arrest equipment in which is incapable of supporting at least 5,000 pounds per employee, there-by exposing employees to fall hazards approximately 120ft. to the ground below. Violation occurred on or about 06/16/16

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 39 Ludlow Street, Newark NJ a) Employer did not provide employees with adequate personal fall arrest systems on the roof of a 12 story building, there-by exposing employees to a potential fall of approximately 120 ft. to the ground below. Violation was observed on or about 6/6/16 Safeway Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 520258, citation number 1, item number 1 and was affirmed as a final order on 10/11/2012, with respect to a workplace located at 800 Washington Rd. Parlin, NJ 08859.
