The service panel was blocked with a table, chairs & water bottles.   Note:  Certification and documentation of abatement must be submitted for this citation item as required by 29. CFR 1903.19(c) and (d), respectively.

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor:  a) On or about  06/17/2016, in front of the IT room corner/ exit path area: Employees were exposed to electrical hazards while using a water cooler which was connected to a flexible extension cord that was not equipped with a grounding conductor.    In the Alternative:    29 CFR 1910.334(a)(3)(iii): Adapters which interrupt the continuity of the equipment grounding connection were used:  a) On or about  06/17/2016, in front of the IT room corner/ exit path area, a water cooler with a three-prong power cord was connected into a two-pronged extension cord with a three-prong/two prong adapter.  Note:   Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
