29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  Location: 234 Clifton Ave, Newark, NJ 07104   a.) Employees engaged in Multifamily Housing Construction were utilizing walkways and floor platforms that had open floor holes leading to the next lower level; there-by exposing employees to fall hazards to the next lower levels and tripping hazards through the uncovered floor holes.   CSHO observed violation on or about 6/04/2016.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: 234 Clifton Ave, Newark, NJ 07104   a.) Employees were engaged in construction activities on a residential roof approximately 30 ft. to the ground below without utilizing any means of fall protection; there-by exposing employees to fall hazards approximately 30 feet to the ground below.     CSHO observed violation on or about 6/04/2016.
